Dog Park Application

By signing below, I acknowledge that I voluntarily apply to use the dog park facilities provided by Magnolia Paws Board & Train. I understand that participation in dog park activities involves risks including, but not limited to, injury to myself, my dog(s), or others. I agree to assume all risk of injury, illness, damage, or loss that may result from use of the park or interaction with other dogs or owners. I certify that my dog(s) are up-to-date on all required vaccinations and are not known to be aggressive or dangerous. I agree to comply with all posted rules and park guidelines. I hereby release, indemnify, and hold harmless Magnolia Paws Board & Train, its owners, employees, volunteers, and affiliates from any and all claims, liabilities, damages, or expenses arising out of my or my dog’s use of the park.
